We finally have a malaria vaccine — Why aren’t children getting it?
After 30 years of development, there is finally a vaccine for malaria. But it might take years to get it to the #children who need it. One year after the World Health Organization approved RTS,S, the first malaria vaccine, only a fraction of the 25 million children who need it have received at least one of the four doses.
Demand is estimated to be 100 million doses per year, but manufacturer GSK can’t produce anything close to that amount. GSK has announced that, by 2028, it will transfer the technology to #manufacture to Bharat Biotech, India. This should help to bolster supplies, but there’s a catch. RTS,S is composed of two parts: the antigen (RTS,S) and an adjuvant called AS01.
Bharat Biotech will manufacture the antigen, but GSK will still supply the adjuvant. AS01 contains saponin - specifically, “a particular saponin you can only extract from a tree called Quillaja saponaria, which grows primarily in Chile”. GSK has promised to supply up to 30 million doses of adjuvant per year. GSK claims that the adjuvant will not be the bottleneck.
And the vaccine, marketed as Mosquirix, has only modest efficacy. On the bright side, researchers say that it has paved the way for better vaccines against a disease that was once considered intractable, with two other candidates currently in trials.
Recently BioNTech, Germany announced plans to launch trials of an mRNA vaccine for malaria by the end of 2022. Another malaria vaccine, being developed at the University of Oxford, called R21, is already in phase III clinicaltrials and, if proven to be safe and effective, it could be deployed as soon as 2023. R21 uses the same antigen as RTS,S, but the results of R21 trials suggest that it might be more effective. If R21 is approved, it could drastically #boost malaria vaccine supplies. Serum Institute of India in Pune has already committed to producing more than 200 million doses per year.

بی بی سی: آزمایش واکسن جدید مالاریا که توسط دانشگاه آکسفورد بریتانیا ساخته شده نشان می‌دهد که این واکسن به مدت دوسال تا ۸۰ درصد موفق به جلوگیری از ابتلاست.این واکسن روی بیش از چهارصد کودک آزمایش شده است. پژوهشگران دانشگاه آکسفورد با موسسه سرم سازی هند که بزرگترین تولید کننده واکسن در جهان است برای تولید این واکسن همکاری می‌کنند.آنان می‌گویند در نظر است حداقل صد میلیون دوز با هزینه کم در سال آینده تولید شود. قرار است این واکسن ظرف چند هفته دیگر جهت بررسی و تایید به سازمان بهداشت جهانی ارسال شود.

رویترز و لنست: داده‌های یک مطالعه فاز میانی روی بیش از ۴۰۰ کودک خردسال که پس از یک رژیم اولیه سه دوزی، دوز چهارم واکسن مالاریای آکسفورد را دریافت کرده بودند، در مجله لنست منتشر شده است. اثربخشی واکسن در گروهی که دوز بالاتری از جزء ادجوانت تقویت‌کننده سیستم ایمنی واکسن را دریافت کردند ۸۰% و در گروه ادجوانت با دوز کمتر ۷۰% در ۱۲ ماه پس از دوز چهارم بود. این دوزها قبل از فصل اوج مالاریا در بورکینافاسو تزریق شدند

هم‌اکنون تنها واکسن تائید شده مالاریا در جهان توسط WHO، واکسن Mosquirix شرکت GSK است. داده‌های مطالعه فاز نهایی واکسن Mosquirix در سال گذشته نشان داد اگر این واکسن قبل از فصل اوج مالاریا در مناطق با سرعت بالای انتقال بیماری تزریق شود، حدود ۶۳% علیه مالاریای بالینی موثر است.
آکسفورد انتظار دارد داده‌های فاز سوم مطالعه واکسن خود را به زودی به WHO ارسال کند و امید دارد این واکسن در سال آینده تائید شود
